The survival horror classic is back, and it’s not letting you escape the nightmare just yet. After everything that happened at Wildberger Hospital, Caroline Walker just wants a normal life with her sister Anna.

That hope doesn’t last long. Anna starts having disturbing visions—death, violence, the works—and soon she’s filling page after page with drawings that feel like they’re leaking into the real world.

Caroline, desperate to help, heads to Villa Hess—a remote mountain town in southern Chile. There’s this odd clinic there, promising answers.

Villa Hess is a maze in every sense—creepy convents, rotting malls, an abandoned school. Every place you visit feels wrong, packed with secrets that refuse to stay buried.

Caroline isn’t powerless, though. She can mess with reality—jump between worlds, change the past, and twist the present to find new ways forward. The line between worlds gets thinner every minute, and the “other side” is always waiting, just out of sight.

Survival’s rough. You’ll piece together weapons from whatever you can grab—nailers, chainsaws, you name it—and switch them on the fly when things get ugly. Ammo and supplies run out fast, the monsters never let up, and every fight has your heart pounding.

Puzzles are everywhere. You’ll dig through items, piece together strange clues, and figure out how everything fits just to move ahead. No one’s handing you answers here—the deeper you go, the more tangled the mystery gets.

Death is always close. Some enemies are quick and vicious, others lumber toward you in heavy armor. You’ll need to stay sharp and think fast, especially when you run into bosses—those fights push you to your limit, and one wrong move can end it all.

The story? It’s dark, layered, and packed with twists. You’ll meet all kinds of people along the way, each with their own secrets, slowly revealing what’s really going on in Villa Hess and what’s haunting Anna.

You can go all-in with the classic survival horror setup—fixed cameras, tank controls, limited saves—or take things a bit easier with assisted mode. The game leaves it up to you: how deep into the nightmare do you want to go?
